    Diagnostic Procedures

    1. Digital Subtraction Angiogram (DSA) of Brain, Head & Neck

    Digital Subtraction Angiogram is a gold standard imaging technique which uses fluoroscopy and contrast media for exhibiting the morphology of blood vessels and diagnosing vascular disorders of the brain, head and neck.


    2. Digital Subtraction Angiogram (DSA) of Spine

    DSA of Spine is an imaging technique which uses fluoroscopy and contrast media for exhibiting the morphology of blood vessels of spine. Gold standard for diagnosing vascular disorders of the spine, DSA of the Spine is performed at very few centres in India with Yashoda Hospital, Nehru Nagar, Ghaziabad being one of them.


    3. Venous Sinus Pressure Monitoring

    A highly specialized procedure, in this,pressure across the venous sinus, mostly the sigmoid and transverse sinus, is monitored, as in cases of idiopathic intracranial hypertension. This is then used to confirm intracranial venous hypertension, which if left untreated, can lead to continuousand severe headaches, blindness and occasionally life risk.


    4. Inferior Petrosal Sinus Sampling (IPSS)

    It is a highly specialised procedure used in cases of Cushing Syndrome, where the adrenocorticotropic hormone (ACTH) levels are sampled from veins that drain the pituitary gland to help diagnose whether the source of overproduction of hormone is pituitary gland or other ectopic source.


    5. WADA Testing

    The Wada test, also known as the Intracarotid Sodium Amobarbital Procedure (ISAP), is used to establish language and memory representation of each cerebral hemisphere.It is performed prior to an epilepsy surgery and provides specific location information about centres within the brain for language, comprehension, recognition, and short-term memory.Currently substituted by non-invasive procedures like fMRI etc., however in cases of equivocal results from other studies, it provides a definitive diagnosis.


    6. Other Non-Vascular Diagnostic Procedures

Therapeutic Procedures

    1. Stroke Management & Mechanical Thrombectomy

    Ischemic stroke is a debilitating condition which occurs due to blockage of blood flow to the brain becauseof clots and can make anyone paralysed for life. Yashoda Hospital, Nehru Nagar, Ghaziabad is among one of the few centres in Uttar Pradesh, providing comprehensive stroke treatment to patients.

    Mechanical Thrombectomy: When these patients present within a window period of 6 hours, the paralysis can be prevented by using a procedure called mechanical thrombectomy. In this procedure, we retrieve the clot from the blood vessel, thus, reestablishing the blood flow. In special cases, these procedures can be performed upto 24 hours.     2. Treatment of Brain Aneurysm

    A brain aneurysm in common language is defined as a bleb or balloon-shaped outpouching in the vasculature of brain which if ruptured can be fatal. Patients with a ruptured aneurysm present as having aSubarachnoid Haemorrhage (SAH) in the brain. Various endovascular procedures which we do in cases of brain aneurysm are as follows:

    a. Coiling: Aneurysm can be treated using a procedure called coiling which is of three types mainly: simple, balloon-assisted or stent-assisted.

    b. Flow diverter: A flow diverter is a pipeline type of device which prevents the flow of blood into the aneurysm, thus preventing its growth and resolution with time.

    c. Other endovascular devices: As aneurysm can be of complex morphology, newer devices such as web devices and contour devices have come into being, to treat them successfully, doing away with the need for open surgery.


    3. Treatment of Vascular Malformations in the Brain, Head & Neck Region

    Vascular malformations in the brain, head and neck region include arteriovenous malformation, arteriovenous fistula, glomus tumors, etc. These lesions can be treated using embolization via specialized embolic materials such as NBCA, onyx or particulate materials.


    4. Treatment for Vascular Malformation of Spine

    Vascular malformation of the spinal cord includesthe arteriovenous fistula and arteriovenous malformation, which can lead to disabling weakness in both legs along with bladder/bowel disturbances. It can be treated using endovascular embolization at Yashoda, making us one of the few centres in the country to provide these services.


    5. Carotid Artery Stenting

    Carotid artery stenosis is one of the major causes of stroke and recurrent stroke. It can be prevented by a procedure called carotid artery stenting which is akin to cardiac angioplasty, but for the brain.     6. Venous Sinus Stenting

    In cases of venous hypertension leading to idiopathic intracranial hypertension, Sigmoid sinus stenting is undertaken. This helps normalise the pressure along the sinus as well as resolution of symptoms.


    7. Mechanical Thrombectomy for Dural Sinus Thrombosis

    In cases of intracranial dural sinus thrombosis leading to brain edema and venous infract, we can retrieve the clot opening the venous sinus leading to faster recovery.


    8. Tumor Embolization

    In case of highly vascular tumors, the blood vessels supplying the tumor can be embolized, which will decrease the blood loss during surgery as well asresult in safer surgeries.


    9. Super Selective Drug Delivery

    In advanced cancer or brain tumor cases, we can deliver the drug directly to the blood vessel supplying tumor, thus leading to a faster response and better outcome.


    10. Vertebral Body Augmentation

    Kyphoplasty and vertebroplasty, the most common forms of vertebral augmentation, can be defined as injecting bone cement, often under fluoroscopy, into a fractured vertebra percutaneously.
